A word document and a separate Excel spreadsheet to accompany it.
Download both. You may be told that your security level won't let you run
Macros in Excel. If that happens, do this:
1. Go to Tools=>Macro=>Macro Security....and set it to Medium Security
2. Save the spreadsheet
3. Close the spreadsheet, then re-open it
4. Tell Excel to enable macros when you are prompted
